Hospice Community Care is our home visiting service which is offered within Hamilton City, Ngaruawahia and Cambridge by our specialist palliative care team, in collaboration with you, your family, your GP and other health providers.
We aim to provide compassionate and skilled palliative care that attends to your physical, social and emotional needs, and the needs of your family/whānau.
This care is primarily provided by our team of specialist staff within your own home. It also includes support if you are in a rest home, as well as outpatient clinics and day hospice services.
Our care includes:
• Assessing your needs and working with you and your family/whānau to develop individual plans of care.
• Help with symptom management.
• Support and education for you and your family/whānau regarding ongoing care requirements.
• Support from a Palliative Medicine Doctor when needed.
• Ongoing emotional, social and spiritual support in collaboration with our Family Services team.
• Communication with other health professionals such as your General Practitioner and/or medical specialist.
• Referral to other services – e.g. home help, personal care and respite care.
• Assisting and coordinating referrals to rest homes or hospital level care facilities if required.
• Loan of medical equipment to aid home care as needed
• Connecting you with skilled support volunteers who are able to spend time listening in a warm and supportive way if you feel this would help.
Hamilton Community Nursing Team
Your first visit will usually be by a Hospice Registered Nurse, who will complete an assessment of your needs, and the needs of your family/whānau.
If necessary, the nurse will refer you to other Hospice services, or in some cases other heath providers.

The Hamilton Community Nurses are available for in-home visits Monday to Friday. There is a reduced service on weekends for urgent needs, as well as an urgent on-call overnight service for patients already under Hospice care.=, seven nights a week.
